Community social support:

Where you are supported in the community by a support worker to help you with the following:
• Grocery shopping.
• Doctor/medical appointments.
• Social outings or activities within the community.
Or alternatively tell us what you need to maintain social inclusion within your community, and we will do our best to accommodate.
 
 
The following services can only be referred by Community Mental Health Services:

Enhanced mobile treatment supports:

• A clinician team works with you and has oversight of medications and supports you with medical appointments. or any social needs that you may require. This service works in partnership with Community Mental Health Te Whatu Ora .

Maternal mental health:

• Support within the home maintaining independence for a clean and safe home environment.
• Supporting mother in maintaining her own mental health and wellbeing with child/baby. This service via ICAMS or Community Mental Health NASC Te Whatu Ora.

Home help supports:

• Assisting with personal hygiene cares and maintaining independence for a clean and safe home environment contact Kia Roha or ACC to access this service or contact us for a private arrangement .
